The form is for routine and planned care: exams and cleanings, a consultation about dentures or implants, a toothache that is manageable, a child's first visit, orthodontic questions, or simply coming back after a long gap. If you are in pain or have swelling, please call instead so we can help the same day where possible.
What to tell us
You do not need to diagnose yourself. A few plain sentences are enough for us to set aside the right kind of appointment:
- The main reason for the visit. For example: "a back tooth has been sensitive to cold for two weeks," "my lower denture has become loose," "I want to find out whether I can have an implant," or "I have not been to a dentist in about six years."
- How long it has been going on, and whether it is getting worse.
- Whether you are a new or returning patient, and if new, the name of your previous dentist so we can ask for records and recent X-rays.
- Your availability. Mornings or afternoons, specific days, and how soon you would like to be seen.
- Anything that would make the visit easier. If you are nervous about dental treatment, need extra time, use a wheelchair, or would like an interpreter or a companion in the room, say so. Our accessibility page describes the assistance available.
Please do not type detailed medical history, medication lists or insurance numbers into the form. We will collect that information securely when we speak with you and through your new-patient paperwork. If you have dental insurance, the insurance page explains what to have ready.

What happens after you submit
- You will see an on-screen acknowledgement that the request was received. This is not a confirmation.
- A member of our front-desk team reviews the request and contacts you by phone or email during office hours, usually the next business day.
- Together you agree a date and time. We will also tell you how long to allow and whether to bring anything.
- Your appointment is confirmed only when we have agreed those details with you. Please do not travel to the office on the strength of the acknowledgement alone, particularly if you are coming from one of the surrounding communities.
If you have not heard from us within two business days, your message may not have reached us; please call.
The new-patient offer is a separate step
The current new-patient exam and cleaning offer is claimed by signing up online through the website. The offer is emailed to you. Signing up for it does not request or book an appointment, and submitting this form does not claim the offer. Emergencies do not use this form
Online requests are read during office hours, so a form is the wrong tool when something is urgent.
- Call 911 or go to the nearest emergency room for difficulty breathing or swallowing, swelling that is spreading quickly toward the eye or neck, bleeding that will not stop, or a serious injury to the face or jaw.
- Call (817) 641-6261 during office hours for severe tooth pain, a knocked-out or broken tooth, a lost crown, or swelling that is localised and not affecting breathing. We keep time for urgent problems where we can.
